All plans are different in regards to deductible, copay, co insurance, and out of pocket max. While most of our services are covered by insurance, you may still be financially responsible if your deductible is not met, or for your portion of co-insurance.

If you have a commercial insurance other than those listed above you can call your insurance provider to see if they will cover services from our clinic. For insurance companies that we are not in network with, we will require payment up front for services and provide you with the diagnosis and procedure codes so that you can submit them to your insurance company for reimbursement.
